Utica University Student Health Portal
Once you are deposit paid and have received your nine-digit student I.D. number, you can register for the Student Health Portal and upload your required health documents. To create your account, please use the link below:
(Important: Portal will open for Fall 2025 students on June 16, 2025.)
Please note:
- All contact with the Student Health Center is strictly confidential. Medical information is released only with students' written permission. MEDICAL RELEASE FORM Must be completed and returned to office for records request.
Students are responsible for notifying their instructors of absences caused by illness or injury. See the Absences section of the student handbook or catalog for information regarding class absences.

Student Health Form
All students must complete and return a HEALTH FORM ( the packet includes a history form, physical exam and immunization page) prior to the start of classes.
