No. If you are submitting official immunizations records, a provider signature is not required. If you are only submitting the health form without additional documentation, a doctor’s signature or office stamp is required.

Yes, as long as all information is clear and legible.
You must receive the second dose and submit documentation. The second dose must be at least 28 days after the first. If you need a second dose, any local pharmacy should be able to provide it.
You can either get another immunization and provide that documentation or you can sign the meningitis waiver on the Health Form declining to receive another immunization and submit that instead.
No, New York State only requires proof of MMR (Measles, Mumps, Rubella) and Meningococcal vaccination or a signed waiver for college students. No other immunizations are required for general compliance. However, additional vaccinations may be required by your specific academic program. You will receive separate instructions from your program if this applies to you.
Yes, all students, regardless of where they live, need to submit immunization documentation if they are attending 6 or more credits of on-ground classes.

You will not be able to move on campus until your records have been received and cleared. You may also be unable to register and attend classes until the requirements are met.
The portal opens after your deposit is paid and you receive your student ID number.
You will receive a confirmation email from the Health and Wellness Center to your Utica.edu email address after your documents are reviewed.
No, Most students do not need to submit a physical exam form. Some programs (such as Nursing, OT, PT and Athletics) will require additional documentation.
You can either contact the Health and Wellness Center to have your portal access reset or you can submit your documentation via email or fax instead.
No, you must be cleared in advance to move in. You will not be able to move onto campus or register for classes until cleared. Please submit documentation by July 1.
